New Patients
Along with your forms, please also bring:
- Your Physical Therapy prescription, if applicable
- Your insurance card or case number
- Any relevant diagnostic reports you may have (for example, X-ray or MRI)
We ask that you also bring a current photo ID with you to your appointment.
As a courtesy to our patients with respiratory problems, allergies and migraines, we ask that you please refrain from wearing any colognes, perfumes or scented lotions during your appointment times with us.
In preparation for your first appointment with Axis, please complete, print and sign the patient forms below.
